

My Aged Care (Support at Home)
​If you or someone you care about wants to keep living at home but is starting to find it harder, occupational therapy can help.
Our experienced occupational therapists work alongside older Australians to support safer, easier day-to-day living. Whether it’s navigating stairs, managing fatigue, preventing falls, or simply finding new ways to stay engaged, our job is to make sure you can keep doing the things that matter most — in the place you know and love.

Support at Home is a government-funded program that helps older Australians live safely and independently
in their own homes. It replaces the former Home Care Package (HCP) model and brings together a wide
range of care and supports — including occupational therapy.

We work closely with Support at Home providers, families, and case managers to deliver practical,
goal-oriented services tailored to each individual.

You don’t need to wait for a fall or a hospital stay to get support. OTs are trained to
identify risk early and provide simple, effective strategies that keep you in control.
We can support you with:
✅ Home safety assessments – identifying hazards and recommending changes
✅ Assistive technology (AT) – mobility aids, toileting support, kitchen modifications and more
✅ Home modifications – from minor adjustments to complex recommendations
✅ Falls prevention – tailored plans to reduce your risk and boost confidence
✅ Daily activity support – making household tasks easier to manage
✅ Fatigue and pain management strategies
✅ Support for memory and cognitive changes – including for people living with dementia
✅ Family and carer education – so everyone feels supported

​​​​​​​Your occupational therapist will come to your home and take the time to get to know you. They’ll ask about what’s going well and what’s feeling harder. Together, you’ll set goals that reflect your priorities — not anyone else’s. You’ll then receive a clear plan and, if needed, a formal report to support access to equipment, modifications or other services through your Support at Home provider. We’ll also work with your care manager or provider directly to make sure everything is streamlined and collaborative.
